When filling out your umpire cards you MUST include the following or you WILL NOT get paid for the game!!!

1.            Date

2.            League

3.            Team NAMES (not color or number)

4.            Legible umpire signature(s)

5.            Winning coaches signature

6.            Score: Both final and per inning

If you cannot see fit to fill them out correctly, I will not see fit to schedule you any more games. PERIOD!!
